Medicaid offices are no longer accept provider birth verification forms, and instead request members to contact the public assistance offices to add their newborn and receive postpartum coverage.
Provider's can still submit birth verification forms in order to help expedite the process by emailing the Public Assistance office directly.
I have included a bare template that includes all of the essential information required on the form and the email to forward your completed forms.
