If you are renewing your professional licenses this year, Medicaid will require a copy of your updated license copies by no later than 12/31/2022 in order to avoid claims processing.
License copies can be faxed to (907) 644-8126, Conduent, ATTN: Provider Services (and include your Medicaid provider ID#)
If you license copies are not turned into Medicaid by 12/31/2022, any claims with dates of service after 1/1/2023 will be suspended without payment until the provider licenses are updated.
